THE WAITING ROOM
The waiting room has been designed for your comfrot and convenience. Please do not smoke. If you have to smoke, please inform the receptionist that you will be waiting outside.
We will endeavor to keep your wait as short as possible. You might be disturbed to see patients called in ahead of you, although you have been waiting longer. These are patients who have been scheduled to be seen by the audiologist spearately. Rendering this service does not interfere with the regular schedule or the time reserved for your.

THE EXAMINATION
The Medical Assistant will bring you to the examining room, where you will be questioned concerning the nature of your illness and your basic medical history. Next, the ears, nose, throat and adjacent structures are examined by me. This is not painful unless these areas are inflammed and tender form your condition.
Following the examination, we attempt to correlate all the information gained and then formulate a plan of treatment. I will explain this to you and prescribe the necessary medications or procedures. If a problem pertains to hearing or dizziness, appropriate tests will be scheduled through our hearing center. My medical assistant may give you additional instructions or printed material.
After your visit is completed, you will be given a charge slip, which shows the fee for the visit and any proposed re-appointments. When you give the slips to receptionist, she will make the necessary appointments and give you a receipt for your payment.
FINANCIAL POLICIES
Because the cost of sending bills and processing insurance claims becomes increasingly more expensive and contributes nothings in the way of quality care, we ask that payment be made for the office visit at the time services are rendered.
We will be happy to submit claims for procedures and sugery, if you will fill out the patient information section beforehand. If you are a member of a pre-paid plan (HMO), please understand that we cannot see you without a referral form from your primary doctor, with the appropriate examination and/or procedures filled in prior to the visit.
Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and not a substitue for payment. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charges. It is your responsiblity to pay any deductible amount, co-insurance, or any other balance not paid by your insurance company. If you have a work or autobmobile related injury and wish your employer or auto carrier to take responsiblity for your bill, please get authorization from them in advance of your visit and bring with you complete information regarding the bill.
If unusual circumstances make payment of your bill difficult, please feel free to discuss this with my staff.
